Job Summary: 
The Department of Pediatrics at the University of Utah School of Medicine has an immediate opening for a part-time Clinical Nurse. The Clinical Nurse works with other team members in a team-based model of care, coordinating, planning, assessing, and evaluating care services to ensure safe and effective patient care.  Additional activities include assisting with clinical triage for our Neonatal Follow-Up Program, quality improvement activities, and performing other duties to facilitate coordination of care for patients.
 
The Clinical Nurse will work with highly skilled team members to provide high value patient care in an organization focused on patient safety, patient and parent satisfaction and positive patient outcomes.  As a team, our objective is to establish and maintain effective processes and collaborative working relationships.

Responsibilities
Essential Functions:
  • Assess patient and family needs for high-risk infant follow-up clinical services; communicate directly and indirectly with healthcare providers to appropriately coordinate all aspects of patient care.
  • Establish early connections with families while infants are still hospitalized in the NICU and CICU, introducing the follow-up programs and building rapport.
  • Coordinate discharge planning between NICU and CICU staff and outpatient clinic teams to ensure seamless transitions of care.
  • Schedule initial follow-up appointments and ensure timely enrollment in the Neonatal Follow-Up Program and Heart Center Follow-up.
  • Maintain and create resources for patients and families.
  • Implement processes to improve patient care and clinic experience - patient tracking and follow-up reminders in EMR (Electronic Medical Record).
  • Educate families about the follow-up program structure, expectations, and importance of developmental monitoring.
  • Serve as primary point of contact for families during the NICU/CICU-to-clinic transition, addressing questions and concerns about outpatient care.
  • Maintain ongoing communication between inpatient providers and outpatient clinic team to ensure continuity of care and information transfer.
  • Proactively identify and address barriers to clinic attendance, including transportation, insurance, scheduling conflicts, and family resource needs.
  • Track and follow up with families who miss initial appointments, providing outreach and support to improve program engagement.
  • Collaborate with social work, case management, and community resources to support families during the transition period.
  • Reach out to community resources to advocate for patients at their schools and in extracurricular environments.
  • Participate in outreach events and educational presentations in the community.
  • Coordinates quality improvement activities.
  • Refer and follow-up on referrals made to psychiatric and medical providers.
  • Facilitate urgent visits for patients outside of provider templates (involves coordination with scheduling, medical assistants, and providers).
  • Coordinate visits with multiple providers in monthly, multidisciplinary specialty clinics.
  • Perform other related duties as assigned.

Minimum Qualifications
Required
Current RQI Healthcare Provider eCredential through the University of Utah Health RQI system. The eCredential is to be obtained within 30 days of hire.
Current license to practice as a Registered Nurse in the State of Utah, or obtain one within 90 days of hire under the interstate compact if switching residency to State of Utah. Must maintain current Interstate Compact (multi-state) license if residency is not being changed to Utah.
Preferred
Some positions may require additional certifications such as Advanced Cardiac Life Support (ACLS), Pediatric Advanced Life Support (PALS), or Neonatal Resuscitation Program (NRP) within one year of hire.
This position is patient-sensitive and must fulfill all associated requirements.
